The RN Clinical Preceptor is a member of the site leadership team and reports to the Advanced Practice Clinical preceptor. This RN works to develop strategies and maintain processes which support the orientation and training of novice and newly-hired clinical staff. Primary responsibility is precepting new and novice clinical staff to become independent and clinically competent members of the team; to ensure understanding of their role and function within the clinical team and to ensure staff are well-prepared to deliver CCA’s clinically-driven, consumer-directed model of care. The RN Clinical Preceptor also trains existing staff and ensures clinical competency of all staff at designated site as needed. Clinical staff may include NPs/PAs, RNs, LPNs & MAs. The Clinical Preceptors maintains a reduced/specified panel of patients for care management.
Monitor the new hire’s work processes and communicate progress to appointed supervisor; case management skills; competence with computer software’s; nurse/patient interactions; clinical competencies, communication with other healthcare providers and within IDT; documentation standards.
Carries a modified panel of patients and assists with managing panels vacated by RNs leaving the site or organization. Uses those panels to train up new RNs and transfers the panel to the new RN upon successful completion of the orientation phase (first 90-days).
Develops and coordinates education experiences and clinical training for novice and newly hired staff on an individual and group basis and monitors progress of CCA Clinical Orientation Competency Program.
Develops individual training plans based on clinician’s needs.
Provides leadership with respect to identification and dissemination of culturally and linguistically appropriate health education. Supports nurses safely practicing to the full extent of their education and training.
Participates in and uses evidence-based research to identify strategies for improving professional development activities, nursing practice and patient centered outcomes.
Ensures educational activities are congruent with CCA’s mission, goals, values, priorities, and resources.
Assists adult learners to identify both their learning needs and the most effective activities to meet those needs.
Provides an appropriate climate for learning and facilitates the adult learning process by developing, planning, and presenting educational activities and directly/indirectly fostering the development of competence in the nursing staff.
With site leadership and HR support, completes 90-day performance evaluations of new RNs, LPNs and MAs and participates in performance management activities as necessary.
Upholds ethical principles in all aspects of practice.
Models behaviors that reflect affiliations with professional organizations as well the community.
Bachelor's Degree or equivalent experience
Master’s degree in nursing strongly preferred
Must be an RN licensed in state of Mass
Experience as a case manager within preferred
Minimum of three years of clinical experience preferred
Home care experience strongly preferred
Must have 3 years recent active employment as an RN
Spanish speaking preferred
Must have at least 1-year experience as a nurse educator or similar teaching experience
Excellent interpersonal skills demonstrated ability to interact professionally with culturally and educationally diverse staff and patients.
Ability to develop plans of care that are individualized to the patient’s condition or needs, reflecting current practice that is evidence-based, providing for continuity of care, and are family centered.
Creativity, flexibility, sound judgment, and the ability to work independently and take initiative.
Excellent time management, documentation and organizational skills.
Highly functioning Computer/software skills, informatics and Microsoft Office programs.
Strong communication and writing skills.
Demonstrate ability to work as an effective team member and leader in complex and fast-paced community environment.
Demonstrated clinical competence in all aspects of primary caring nursing, care management, nursing assessment, care planning, and medical/surgical nursing.
Strong desire to teach
