Social Services Care Coordinator – Empower, Support, and Transform Lives
Are you passionate about making a real difference in people's lives? The Social Services Care Coordinator is a vital member of the multidisciplinary treatment team, working directly to support the development and implementation of personalized treatment plans. This role focuses on discharge planning and facilitating smooth transitions for individuals served. As a compassionate advocate, you’ll assess psychosocial needs, provide case management, and connect individuals with valuable community and facility resources. You’ll also work closely with families, offering education and support while ensuring timely, compliant documentation. Your efforts will directly contribute to enhancing recovery outcomes and overall well-being for those you serve.
Key Responsibilities
Interviews for Background Information : Interviews individuals, families, and other stakeholders to gather background information and social history, which is essential for creating an accurate treatment plan.
Discharge Planning : Participates in discharge planning activities, and may be responsible for establishing the discharge plan, ensuring smooth transitions from the facility.
Arranges Aftercare Services : Coordinates aftercare services and community linkages, communicating with internal and external systems to support individuals served in their post-care needs.
Therapeutic Activities : Provides therapeutic activities as assigned, contributing to the individuals' recovery and well-being.
Multidisciplinary Team Participation : Actively participates in multidisciplinary treatment team meetings, contributing to the development and implementation of treatment plans.
Qualifications & Requirements
Education:
Required: Bachelor's degree in social work, counseling, psychology or related field.
Preferred: Master’s degree, unless required by contract or state.
Experience:
- Required: Minimum of relevant professional experience and/or training with relevant population.
Licenses:
- Preferred: Licensure appropriate to discipline.
