The Cambridge Health Alliance is looking for a dynamic speciality clinical pharmacist to join our dedicated team at our Ambulatory Pharmacy Department located in Cambridge/East Cambridge/Malden/Revere. Our ambulatory pharmacies are embedded within the health system, providing opportunities for interprofessional collaboration. Our Ambulatory Pharmacy Team utilizes a top notch pharmacy system and state of the art automation. We utilize EPIC for our Electronic Medical Record (EMR), Parata/RapidPak automation for dispensing, and Google Workplace in our Ambulatory Pharmacies. The services we provide are performed with a focus on optimized patient care and safety, increasing patient and provider satisfaction, and enhancing the financial sustainability of the organization. Job Purpose: Regularly provides clinical specialty pharmacy services via direct patient care in ambulatory clinics or the outpatient pharmacy. Monitors specialty drug therapy using implicit and explicit criteria to ensure safety and effectiveness. Performs formal and informal medication use consults with appropriate documentation. Recognized by colleagues as a medication use expert in specialty area(s). Provides direct, effective, and individualized education and counseling to patients and/or family members/caregivers. Assesses need, develops and/or refines clinical pharmacy programs. Assesses clinical and financial impact of efforts on an ongoing basis. Evaluates medication use (i.e., formal usage evaluations); effects measurable improvements in medication use process. The specialty clinical pharmacist is responsible to ensure that periodic consultations with providers occurs when needed The specialty clinical pharmacist is responsible to ensure that qualified clinicians are accountable to the organization for decisions affecting consumers.

Education/Training: Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree is required. Board Certification in Specialty Pharmacy is required (to be completed within two years of hire).

Licensure: Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA. In lieu of an active MA license, candidates must attain MA licensure prior to working 1000 hours at the Cambridge Health Alliance (CHA). Failure to attain MA licensure within this time period will result in the discontinuation of employment at CHA.

Work Experience: Minimum of three (3) years’ experience as a pharmacist, five (5) years preferred.
