Job Title: Triage Nurse

Responsibilities

• Complete MassHealth Application and Evaluation for PCA Services in compliance with MassHealth regulations.

• Meet deadlines for completion and submission of assessments.

• Collaborate with Occupational Therapists on Evaluations as needed.

• Work with Functional Skills Trainers and the PCA Program Director to ensure consistency and compliance of all Evaluations before submission.

• Participate in staff trainings and meetings.

Essential Skills

• Ability to multi-task and work independently as well as part of a team

• Accurate patient charting

• Ability to be patient and work well with geriatric patients

• Chart review

• Healthcare

Additional Skills & Qualifications

• Active Massachusetts LPN or RN license

• At least 1 year of clinical experience in a community health or short term care setting

Pay and Benefits

The pay range for this position is $32.00 - $32.00/hr.
