HEALTH SERVICES SUBSTITUTE

We are looking for a Health Services Substitute to join our team! The Health Services Substitute

will be responsible for the maintenance of administrative requirements of assigned children

and assuring proper follow up is completed. This position will also be working in support of an

interdisciplinary Case Management Team that will aid in connecting families with ongoing

health care.

RESPONSIBILITIES

The Health Services Substitute will perform routine health services including, but not limited to:

? Reviewing applications for current health documentation prior to child?s enrollment.

? Assuring that all of the health requirements for the assigned case load remain current

through tracking, timely notification, and follow up with families.

? Administering auditory, visual and other health screenings within 45 days of a child?s

enrollment.

? Preparing and maintaining health services records, assuring that services are properly

documented in Case Management Files.

? Prepare monthly reports; assist the HND manager as assigned in the implementation of

service plans, procedures, schedules, and other health component activities.

? Enforce health and safety practices at the sites; and provide support services in cases of

suspected child abuse/neglect as part of the Case Management Team.

? The Health Services Substitute will support school readiness activities and actively

promote parent engagement, assist with cross-component duties as assigned based on

program needs.

QUALIFICATIONS

? Must be CNA or LPN and have relevant work experience in the health or social services

field.

? Must have experience working with low-income families from varying cultural

backgrounds.

? Experience with homeless, special needs children, adults, and their family is a plus.

? Must have strong written and oral communication, interpersonal, organizational, and

time management skills and be able to work within a case management team.

? Must possess a valid driver?s license and means of daily transportation.

? Bilingual English/Spanish preferred (American Sign Language is a plus), and exhibit

excellent analytical and computer skills.

? Flexible schedule is required to meet the needs of our children and families; including

some evenings/weekends.

? Ability to read, write, and express oneself in English required.

? CPR and First Aid certificate preferred or ability to obtain CPR/ FA within 30 days of hire.

? Show documented proof of current physical exam, negative TB test and proof of MMR

immunity. (Hepatitis B immunity is advisable).

? Must have a suitable CORI, SORI & DCF background check on file prior to employment.

? Must have a suitable fingerprint check completed within the first thirty (30) days of

employment.

? Travel is required for home visits, meetings, training, and off-site events.

? This position is very active and requires standing, walking, bending, kneeling, stooping,

crouching, crawling, and climbing all day. The employee must frequently lift and/or

move items over 50 pounds.
