Wellforce Nurse Navigator in Medford, Massachusetts
Position Summary: The Nurse Navigator is a certified nurse who guides patients through the process of diagnosis, monitoring and treatment in the detection of pulmonary nodules. The Nurse Navigator will coordinate each step of the care pathway for patients. Through work achieved within the Radiology Department, the Nurse Navigator improves the care experience, and provides patient-focused clinical triage, coordination, and follow-up services that assure timely and seamless integration of care within Radiology and across other departments. There are two programs that the Nurse Navigator will focus on: (1) Lung Cancer Screening program & (2) Incidental Pulmonary Nodule Program. Working in close collaboration with the Radiology team and providers, the Nurse Navigator will work in the implementation of these programs within the MelroseWakefield Healthcare.
EDUCATION: Graduate of an accredited school of nursing. BSN required. Registration and current RN license required. CPR certification required. I.V. certified. BLS certification required. Will consider extensive experience in a clinical setting in lieu of education requirement.
EXPERIENCE: Two (2) to three (3) years of clinical experience in a patient care setting required. Experience in pulmonary or oncology helpful.
SKILLS/ABILITIES: Good interpersonal and organizational skills. Ability to work with detailed, confidential material, computer experience required. Exercises good clinical judgment with the ability to recognize the entire plan of care focusing on the whole person and not just a specific component of need. Proficiency with Microsoft Office products, specifically Excel, required. Ability to work independently and meet deadlines. Excellent oral and written skills are req
